Cancer Council Victoria
To find a publicly funded DE/CDE use the search function of the Human Services Directory www.humanservicesdirectory.vic.gov.au 
Patients with a chronic condition who are managed under a GP Management Plan (GPMP) and a Team Care Arrangement (TCA) can receive a Medicare rebate if referred to a credentialled diabetes educator using the 'EPC' form.  Patients witha GPMP/TCA can also be referred to a DE at your local community health service using the VSRF or a service-specific referral form.
