impetus™ encourages clinical relationships between public and private allied health providers and GPs. The emphasis is on improving communication systems, integrated care planning, and access to diabetes services.

Our work with the Primary Care Partnerships is designed to strengthen relationship between public allied health providers and GPs.

We have a number of activities designed to assist general practices collaboration with private allied health providers under Enhanced Primary Care. The Chronic Disease Management (CDM) MBS initiative allows patients with a chronic condition, managed by a GP Management Plan & Team Care Arrangement, access rebates to Medicare registered services.

Our range of services to support linkages between allied health providers and GPs are:

  • Aged Care Access
  • ATAPS
  • Diabetes Prevention
