impetus™ is involved in two distinct but similar diabetes prevention programs. Life! Taking Action on Diabetes is a lifestyle modification program (LMP) for patients over the age of 50, at high risk of type 2 diabetes. Life! is an accredited LMP course, provided by Diabetes Australia-Victoria (DA-V) using accredited Life! providers. Diabetes educators interested in becoming an accredited provider should contact Diabetes Australian – Victoria. Lifestyle Modification Program is an individual or group education program to support 40-49 year olds make healthy lifestyle changes to prevent or delay the onset of Type 2 diabetes. It is based on national LMP standards and potential providers must be accredited before they can deliver services and receive a Commonwealth subsidy. Patients are referred by GPs. Accredited providers who wish to provide services within our catchment should register with General Practice Victoria.
