This website provides resources to assist general practitioners (GPs), staff in Residential Aged Care Facilities (RACF), and other providers of medical care for residents ‘round the clock’.
In the ‘GP and Residential Aged Care Kit’, Section 1 describes how to use the kit.
The service system and partnership approach is outlined in Section 2.
Section 3 and Section 4 contain tools that GPs, RACF staff and other services can use.
Section 5 provides information on Clinical Information Sheets and Reference Cards.

The resources are designed to:
Be adapted and used across high-level and low-level care facilities
Streamline communication and documentation for routine medical care, medication management, after hours care, resident transfer to and from hospital, and advance care planning
Optimize the prevention and management of conditions that commonly occur ‘round the clock’
Comply with privacy legislation for use of health information in the care of RACF patients
Be used within current service system and funding arrangements.
Resources were produced by the ‘After Hours Primary Medical Care for Residents in Residential Aged Care Facilities Project’ (2004) and ‘Resources for After Hours Primary Medical Care in RACFs Project’ (2006), under the auspices of North West Melbourne Division of General Practice (NWMDGP). A needs assessment study analysed service utilisation patterns and the factors influencing what happens when a resident needs medical attention after hours. Resources were developed and piloted in partnership with RACF staff, GPs, medical deputising services, Metropolitan Ambulance Service, local hospitals, Divisions of General Practice, and consumers in the northwest area of Melbourne (see Acknowledgements).
TThe Australian Government Department of Health and Ageing funded the needs assessment in 2001, the service development grant from January 2003 to December 2004, and the extension of service development grant from December 2005 to November 2006. Melbourne Health Human Research and Ethics Committee approved the service development project and its evaluation.
